Write times with am or pm, or use the 24-hour clock, so nobody has to guess. For each day, total hours = time out − time in − the unpaid lunch.
Write totals in decimal hours if your payroll asks for them. Say a day runs from 8:00 am to 4:45 pm with a 30 minute unpaid lunch: that is 8 hours 15 minutes, or 8.25 decimal hours, not 8.15. The decimal hours calculator converts any total for you.
Yes. Click Print, then choose Save as PDF (or Microsoft Print to PDF) as the printer. Keep the margins on Default and the scale at 100% so the sheet fits the page.

Total each week separately. Under the US federal rule, overtime is for hours over 40 in a single workweek, so two weeks can't be averaged together.
